fileUpload inside formular

I thought, that I have a real common task to to, but I really found no solution or hint.

I need to build a fromular with some inputFields for data, lets say to specify some values: one a date, one a number anf one for a comment text.
additionaly I need to upload files together with these data. These are information concerning the uploded files. On serverside I will store the files on server an need to store the other data in a database together with the files and there folders where I put them.

How can I submit all data together, e.g. with a command button, doing validation and then submit all files and the values.

I cannot believ that i an the only one with such task.
