Update guide from UltimaNG 1.1.0 to 2.0.0

I updated my UltimaNG 1.1.0 CLI project to 2.0.0 by following the steps in the documentation.

I felt that the steps are somehow incomplete. For example

- Not only layout.js was removed from layout/js directory, but also all other JS files except ripple.js
- I had to add nanoscroller to package.json
- I had to move the new app.menu, topbar, ... files as well into my project beside app.component
- I had to replace the layout/css folder as well, because I had some visualization issues on the menu
- ...

It would be great to have a complete upgrade guide in the future, which would make migration much easier.

- Also in the release there are some .DS_Store files, which are OS specific files as far as I know.
- It would be great, if the code could be improved to pass ng lint without any issues

I'm starting migration now and it would be really helpful to know a full list of what you had to do.

What kind of information do you require more, I believe there is room for improvement as well.

I also feel that documentation could be improved. Here's one thing I have noticed:

ripple.js requires jQuery, but both are labelled "optional" scripts, and no co-relation is noted in the documentation.
--> Either refactor ripple.js to remove jQuery, Or, note in the documentation that jQuery is also needed for ripple.js.
